According to the Associate Vice President (AVP) of Quality Assurance, the (b)(4) Block (b)(4) Production Office (Room NO. (b)(4)) is routinely used to print and review alarm and audit trail reports of production equipment involving (b)(4) products. The printed reports are signed off and then attached to final batch production records. On January 28, 2026, an unplugged computer system was observed to be backdated to November 3, 2025, upon start-up. According to the Senior Executive of Production, the computer system has a shared username and password for the department. In addition, the same computer has permissions for deleting files and permanently delete files from the computer recycle bin. There is no documented explanation for the need for production officers to have deletion controls and access to additional applications, including a raster graphics editor for image creation and editing.
